Program Coordinator

We are seeking a compassionate, organized professional to coordinate community habilitation services for individuals with developmental disabilities. In this role, you will connect people, families, Direct Support Professionals (DSPs), and service teams—making sure each person’s goals remain at the center of their support plan.

This is a hands-on coordination and staff-supervision position for someone who believes in choice, inclusion, and helping people build meaningful lives in their communities.

What you’ll do

· Recruit, interview, train, schedule, and directly supervise Direct Support Professionals.

· Coordinate individualized habilitation services and help develop Staff Action Plans based on each person’s goals.

· Monitor services, progress, and required documentation to ensure quality and compliance with agency, OPWDD, and Medicaid requirements.

· Review DSP timesheets, service notes, mileage, and expense requests for accuracy and completeness.

· Participate in intake activities, DSP/consumer meet-and-greets, and six-month and annual Life Plan meetings.

· Build positive working relationships with individuals, families, staff, and members of the habilitation team.

· Help expand opportunities for independence, daily living skills, recreation, social connection, and community inclusion.

· Respond to and report incidents in accordance with established policies and regulatory requirements.

What you’ll bring

· Bachelor’s degree in Human Services or a related field.

· At least one year of experience working with individuals with developmental disabilities.

· Experience recruiting and supervising staff.

· A strong commitment to independent living, person-centered planning, consumer choice, and full community inclusion.

· Excellent communication, organization, time-management, and documentation skills.

· Proficiency with Microsoft Word, Excel, PowerPoint, and other routine computer-based documentation.

· A valid driver’s license, reliable vehicle, and ability to travel throughout the Southern Tier as needed.

· Ability to work occasional evening or weekend hours based on individual and program needs.
