This is an excellent opportunity for a nutrition professional who is passionate about improving patient health outcomes through comprehensive nutritional assessment, education, and intervention.
Responsibilities
Conduct comprehensive nutrition screenings and assessments.
Develop and implement individualized Medical Nutrition Therapy (MNT) plans.
Document nutrition assessments, interventions, and outcomes within the electronic medical record.
Provide nutrition education, counseling, and group sessions for patients, families, and caregivers.
Participate in multidisciplinary treatment team meetings and patient care rounds.
Support facility-wide Quality Assurance and Performance Improvement initiatives.
Contribute to staff development and clinical outcome improvement efforts.
Complete swallowing screenings as needed.
Collaborate with physicians, nursing staff, and other healthcare professionals to ensure coordinated patient care.
Qualifications
Bachelor's or Master's Degree in Clinical Nutrition, Dietetics, or a related field from an accredited institution.
Current Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) credential required.
Strong assessment, counseling, documentation, and communication skills.
Experience working in a healthcare, behavioral health, long-term care, or institutional setting is preferred.
Required Health Documentation
Candidates must provide:
Proof of MMR immunization (titers may be required if immunizations are more than 10 years old).
Current PPD/TB screening completed within the past year.
Flu vaccination preferred.
